Hospital discharge survey

Aftercare is important and that’s why this month the health charity Healthwatch are carrying out a special enquiry into discharge from hospital.

In order to get as wide a range of opinions as possible before Healthwatch report back on the 31 July they are asking for anyone who has been discharged from hospital or any other health unit to complete a quick online survey.

The survey is open to anyone from Bristol, Bath and North East Somerset, South Gloucestershire and Somerset and can be found at the following address: https://tinyurl.com/o783uqg
